Job summary

Graham Healthcare Group is seeking a Home Health Physical Therapist for a full-time role in Monessen, Pennsylvania. Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college.
• Licensed Physical Therapist in the state you will work.
• Minimum of one year healthcare experience.
• Valid driver’s license with ability to travel ~20% of each day.
• Ability to meet critical deadlines for charting and self-manage time.
• Experience using EMR software and tablets/mobile devices.

Responsibilities

• Collaborate with the care team to manage and deliver patient care plans.
• Provide home health physical therapy services to assigned patients.
• Document progress and adjust treatment plans in the EMR.
• Support scheduling and coordination of patient visits as needed.
