Qualifications

• Bachelor’s or Master’s in Physical Therapy from an accredited college.
• Licensed Physical Therapist in the state where you will work.
• Minimum of one year healthcare experience.
• Driver’s license and ability to drive ~20% of your day.
• Must meet charting deadlines and manage time effectively.
• Demonstrated experience with tablets, mobile phones and EMR software.

Responsibilities

• Deliver management and delivery of patient care plans within a team-based home health setting.
• Collaborate with the care team to provide high-quality patient care and ensure timely documentation.
