BAYADA Home Health Care is seeking a Home Health Physical Therapist for Maple Shade, New Jersey, offering consideration for full-time positions with home health and outpatient visits across Camden and Burlington Counties. This role emphasizes high-quality 1:1 patient care and flexible scheduling.

Qualifications

• Current NJ Physical Therapist (PT) license.
• Graduation from an APTA-accredited program or AMA equivalent with transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, balance, gait, strength, endurance).
• Implement wound care and protective interventions.
• Develop and revise individualized care plans with PT interventions.
• Educate clients and families on rehabilitative care and safety for independent living.
• Document observations and interventions accurately in EMR.
