Job summary

Adoration Health in Waycross, GA is seeking a dedicated Physical Therapist for home health. You will conduct patient evaluations, design individualized treatment plans, and work collaboratively with a care team to support patient independence.

Responsibilities include documentation, coordination with physicians and families, and supervising support staff as needed. Requires state licensure and CPR certification, with opportunities for growth and benefits in a compassionate, patient-centered

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• At least 2 years of physical therapy experience; hospice/community experience preferred.
• Valid license to practice physical therapy in the state.
• Current CPR certification.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including adaptive equipment recommendations.
• Develop measurable goals and treatment plans to move patients toward independence.
• Maintain ongoing communication with the treatment team, patient, and family.
• Document services thoroughly for proper billing.
• Supervise PTAs and PT aides per state practice acts.

Education

Bachelor's Degree in Physical Therapy

2+ years of experience

CPR certification

Licensed in state of practice
