Job summary

Adoration Home Health in Vidalia, GA is seeking a dedicated Home Health Physical Therapist to provide patient-centered care in a 1:1 setting. The role offers a full-time schedule and opportunities for growth within a supportive team.

Qualifications

• Must have a Bachelor's degree in Physical Therapy.
• New graduates may be considered in select markets.
• At least 2 years of clinical experience; hospice/community experience preferred.
• Valid license to practice Physical Therapy in the state.
• Current CPR certification.

Responsibilities

• Perform PT evaluation and treatment per physician orders and adaptive equipment.
• Establish measurable goals and treatment plans to maximize independence.
• Maintain communication with the treatment team, physician, patient, and family.
• Document services thoroughly for proper billing.
• Supervise PT Assistants and PT Aides in patient care per state practice act.
