Job summary

Adoration Health in Vidalia, GA seeks a dedicated Home Health Physical Therapist to deliver patient-centered care in a home setting. This full-time role offers a $10K sign-on bonus for external candidates and a schedule that supports work-life balance.

You will evaluate and treat patients, develop goals, coordinate with physicians and families, supervise Aides as needed, and ensure proper documentation for billing. Eligible for comprehensive benefits.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New physical therapy graduates may be considered in select markets based on program availability.
• At least 2 years of appropriate experience as a physical therapist; community/hospice experience preferred.
• Valid license in the state of practice; current CPR certification.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based on physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward independence and function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided for proper billing
• Supervise PT Assistants and PT Aides in direct patient care per state act
