Job summary

Adoration Health in Nashville, TN is seeking a dedicated Home Health Physical Therapist to provide individualized PT services in the patient’s home. You will evaluate patients, design treatment plans, and work with a multidisciplinary team to support recovery and independence.

Qualifications

• Graduate of a Physical Therapist Education program approved by CAPTE.
• Current Physical Therapist licensure in State and CPR certification.
• Two years of experience in adult physical therapy; home health experience preferred.
• Excellent observation, verbal and written communication, problem solving and basic math skills.

Responsibilities

• Perform PT evaluation and treatment based on physician orders and adaptive equipment needs.
• Develop measurable goals and treatment plans to maximize independence.
• Maintain communication among treatment team, physician, patient and family as applicable.
• Document services thoroughly and timely for proper billing.
• Supervise PTAs and PT aides per state practice act.
