Adoration Health is seeking a dedicated Home Health Physical Therapist Assistant to join our team in Lafayette, TN. You will provide patient-centered, one-on-one PT services under supervision, ensuring progress toward goals in a home-health setting.

Qualifications

• Current unrestricted license as a Physical Therapist Assistant in the practicing state.
• Current Basic Life Support/CPR certification in accordance with state regulations.

Responsibilities

• Provides physical therapy services planned, delegated and supervised by the Physical Therapist.
• Confers with Physical Therapist regarding plan of treatment and progress.
• Communicates patient needs and progress to PT and assists with adaptive equipment as needed.
• Maintains and submits patient treatment and billing documentation as required.
