Adoration Health is seeking a Home Health Physical Therapist to join our Meridian, MS team. You will provide 1:1 patient-centered care in a supportive environment, with scheduling options that promote balance.

The role requires CAPTE-approved PT education, state licensure, CPR, and at least two years of adult PT home health experience. You’ll work Monday–Friday, 8a–5p, with a mix of independent and team collaboration.

Qualifications

• Graduate of a Physical Therapist Education program approved by CAPTE.
• Current Physical Therapist licensure in State and American Heart Association CPR certification; CPR may be offered upon hire.
• Two years of experience in adult physical therapy; home health experience preferred.
• Excellent observation, verbal and written communication skills, problem solving, basic math skills; PT skills per competency checklist.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including adaptive equipment recommendations.
• Establish measurable goals and treatment plans to maximize independence.
• Maintain communication among treatment team, physician, patient, and family as applicable.
• Maintain thorough and timely documentation for proper billing.
• Supervise PTAs and PT Aides in direct patient care and related activities per state act.
