Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a dedicated Physical Therapist to join our Home Health team in Spring Hill, TN. You will provide expert, patient-centered care in a home setting, helping patients regain independence and improve functional outcomes.

Qualifications

• Graduation from a CAPTE-approved Physical Therapist education program.
• Current State Physical Therapist licensure is required.
• American Heart Association CPR certification is required (may be offered on hire).
• Two years of adult physical therapy experience; home health experience preferred.

Responsibilities

• Perform physical therapy evaluation and treatment based on physician orders, including adaptive equipment recommendations.
• Establish measurable goals and develop treatment plans to maximize independence.
• Maintain communication among the treatment team, physician, patient, and family.
• Maintain thorough and timely documentation for proper billing.
• Supervise PTAs and PT aides in direct patient care per state practice act.
