Interim HealthCare is seeking a dedicated Home Health Physical Therapist to support patients in Jacksonville, FL and surrounding areas. You’ll provide home-based therapy to improve mobility, strength and pain management, following a patient-centered plan of care created with a clinical team.

The role requires Florida PT licensure, CPR certification, and at least 1 year of PT experience, preferably in home health. Competitive salary, mileage reimbursement, and generous benefits are offered.

Qualifications

• Graduate of an accredited Physical Therapist program with active Florida license.
• Minimum 1 year of physical therapy experience, ideally in home healthcare.
• CPR certification is required.

Responsabilités

• Provide home-based physical therapy to patients with mobility and strength challenges.
• Collaborate with home health team (RN, LPN, CNA, HHA, OT, SLP) on plans of care.
• Observe movements, document progress, and adjust care plans as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients and families on exercises, goals, and safety.
