Job summary

Interim HealthCare of Mercer County is seeking a Home Health Occupational Therapist (OT) to deliver individualized therapy in patients' homes across New Jersey. You will design and implement customized treatment plans, monitor progress, and adjust goals with a focus on improving daily living activities in a home environment.

The role emphasizes patient-centered care, collaboration with a multidisciplinary team, and adherence to home health regulations.

Qualifications

• Graduated from an accredited Occupational Therapy Program and must hold an active New Jersey OT license.
• Minimum 1 year in occupational therapy, ideally in home health.
• CPR certification within 60 days of hire.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks due to illness or injury.
• Collaborate with a home health team (RN, LPN, CNA, HHA, PT, SLP) to follow the plan of care.
• Assess patient, observe deficits, set therapy goals and document progress.
• Assist patient with exercises to improve fine motor skills and coordination.
• Suggest adaptive equipment (grab bars, shower chairs) for added support.
• Assess fall risks and introduce home-safety strategies.
• Educate patient and family on plan of care, exercises and self-care.
