Job summary
Interim HealthCare of Mercer County is seeking a Home Health Registered Nurse (RN) in New Jersey. You will provide in-home care, coordinate with a physician team, and educate patients and families on plans of care and safety.
Join a supportive, autonomous role with flexible scheduling designed to balance work and life. The ideal candidate has an NJ RN license, 2+ years in home health, CPR certification, and a nursing degree with a preference for a BSN.
Qualifications
- Graduate of an accredited nursing program.
- Active RN license in NJ.
- Minimum 2 years nursing experience, ideally in home health care.
- CPR certification and knowledge of home health regulations.
Responsibilities
- Provide home-based care to patients with illnesses, injuries and chronic diseases.
- Work with a team of physicians and oversee LPNs and CHHAs providing patient care.
- Examine patients, perform assessments, document progress and report changes to physicians.
- Educate patients on plan of care, medications, therapy, diet and exercise.
- Coach family members on care plans and home safety.
- Coordinate discharge from services and ensure goals are met.
