Job summary

Elara Caring is seeking a PRN Home Health Registered Nurse in Harker Heights, TX, to deliver skilled nursing care in patients' homes. This part-time, field-based role offers autonomy, one-on-one patient interactions, and the chance to impact patients' independence and safety at home.

You’ll perform admissions, visits, recertifications, and care plan updates, monitor conditions, educate families, and complete documentation while traveling within your territory.

Qualifications

• Current, unrestricted Texas RN license.
• Previous home health nursing experience required.
• Ability to work independently and manage visits efficiently.
• Willingness to travel throughout territory.
• Valid driver’s license and auto insurance.

Responsibilities

• Complete admissions, assessments, routine visits, recertifications, and discharges.
• Develop and update individualized plans of care.
• Monitor patients' conditions and communicate significant changes.
• Coordinate care with physicians, caregivers, and the interdisciplinary team.
• Educate patients and families about medications, conditions, safety, and self-care.
• Complete accurate and timely clinical documentation.
• Help prevent avoidable hospitalizations through proactive nursing care.
