Job summary

Signature Bend Home Health seeks a Part-Time or PRN Physical Therapy Assistant to deliver in-home physical therapy services under supervision. You will implement individualized plans to improve strength, mobility, balance, gait, and overall function while traveling across the service area.

You will educate patients and families, document progress, and collaborate with an interdisciplinary team to ensure safe, effective care. A valid driver's license and ability to travel are required.

Responsibilities

• Provide physical therapy treatments in patients' homes under the supervision of a Physical Therapist.
• Implement individualized treatment plans to improve strength, mobility, balance, coordination, transfers, gait, and overall function.
• Monitor patient progress and communicate changes to the supervising Physical Therapist.
• Educate patients, families, and caregivers on exercises, safety techniques, and assistive devices.
• Maintain accurate, timely clinical documentation.
