Job summary

Signature Bend Home Health is seeking a part-time/PRN Physical Therapy Assistant to provide in-home rehabilitation services in Bend, OR under the supervision of a Physical Therapist. You will implement individualized treatment plans, monitor progress, and communicate findings to the supervising PT to optimize patient outcomes.

Qualifications

• Provide physical therapy treatments in patients’ homes under the supervision of a Physical Therapist.
• Monitor patient progress and communicate changes to the supervising Physical Therapist.

Responsibilities

• Educate patients, families, and caregivers on exercises, safety techniques, and assistive devices.
• Maintain accurate, timely clinical documentation.
• Travel throughout the service area to provide in-home patient care.
• Ability to lift up to 40 lbs., assist with transfers and mobility activities, and navigate stairs and varying home environments.
