Trinity Health At Home – Muskegon is seeking a Home Care Physical Therapy Assistant (PTA) to deliver physical therapy services under a Physical Therapist's supervision, following a physician and therapist plan of care, in patients' homes across the Grand Haven/Spring Lake area.

Qualifications

• Licensed PTA in Michigan with current certification.
• Graduation from a two-year college-level PTA program approved by the APTA.
• Minimum six months experience under a qualified Physical Therapist.
• Home health experience preferred.
• Must have reliable transportation to the work site.

Responsibilities

• Provide physical therapy services under supervision and in accordance with a treatment plan.
• Deliver quality patient care with 1:1 sessions in home health settings.
• Assist with documentation and coordination of care.
• Maintain professional relationships with clients and families.

Minimum Qualifications:

• Licensed PTA within the State of Michigan
• Graduation from a two-year college-level program approved by the American Physical Therapy Association.
• Minimum of six months experience under a qualified Physical Therapist.
• Home health experience preferred
• Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the agency’s clients and their families.
• Must have current Driver’s license and reliable transportation to and from work site.
