Job summary

Millennium Physician Group is seeking a Physical Therapy Assistant (PTA) to join our Home Health Team in Charlotte County, Florida. You will provide skilled therapy services under the supervision of a Physical Therapist to help patients improve mobility, function, and independence in their homes.

Responsibilities include implementing treatment plans with exercises, gait training, and device training; educating patients and families; collaborating with an interdisciplinary team; and maintaining

Qualifications

• Florida PTA license in good standing.
• Minimum 6 months experience in hospital, skilled nursing facility, or rehab.
• Excellent communication, interpersonal and critical thinking abilities.
• Strong work ethic and reliability; patient-centered care.
• Reliable transportation to visit patients’ homes.

Responsibilities

• Provide skilled therapy services under the supervision of a licensed Physical Therapist.
• Implement treatment plans including therapeutic exercises, gait training, and assistive device training.
• Educate patients and families to support recovery and daily living activities.
• Collaborate with PTs, OTs, and other health care professionals to optimize outcomes.
• Maintain accurate and timely documentation of patient progress and plans.
• Adhere to infection control guidelines to protect patients and staff.
