Job summary

Millennium Physician Group is seeking a Physical Therapy Assistant (PTA) to join our Home Health Team in Collier County. You will provide skilled therapy under the supervision of a Physical Therapist to help patients improve mobility and independence in their homes.

Responsibilities include implementing treatment plans with therapeutic exercises, gait training, patient and family education, and collaboration with an interdisciplinary care team to ensure quality outcomes.

Qualifications

• Licensed PTA in Florida.
• Minimum of 6 months in a hospital, skilled nursing facility, or rehab.
• Excellent communication, interpersonal, and professional demeanor.

We are seeking a Physical Therapy Assistant (PTA) to join our Home Health Team in Collier County. In this role, there is a real impact in the comfort of patients’ homes. As part of our compassionate, high‑performing care team, you will provide skilled therapy services under the direction and supervision of a Physical Therapist to help patients improve mobility, function, and independence. You will implement individualized treatment plans, including therapeutic exercises, gait training, and assistive device training, while educating patients and families on techniques that support recovery and daily living. You will collaborate with an interdisciplinary care team, maintain accurate clinical documentation, and adhere to infection control and safety standards to ensure high-quality patient care and optimal outcomes.

This is a Pro Re Nata (PRN) position with variable time and hours can vary from week to week based on staffing needs, patient volume, call-offs, vacations, or organizational requirements.

• Skills: Excellent communication, interpersonal, and critical thinking skills.
• Professionalism: Strong work ethic, reliability, and a commitment to providing quality care.
• Transportation: Reliable transportation to visit patients in their homes.
