Job summary

Millennium Physician Group in Florida is seeking a Physical Therapist to join our Home Health Team serving Charlotte County. You will evaluate patients, develop individualized treatment plans, and provide therapy to improve mobility, strength, balance, and functional independence in patients' homes.

This PRN role offers flexible hours; candidates must hold a Florida Physical Therapist license, have at least 1 year of clinical experience, and demonstrate strong communication and collaboration

Qualifications

• Current and valid Physical Therapist license in Florida.
• Minimum 1 year of clinical home health experience preferred.
• Excellent communication and interpersonal skills; patient-centered approach.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Provide manual therapy, therapeutic exercises, and gait training to improve mobility and strength.
• Educate patients and families on home exercise programs and injury prevention.
• Maintain accurate documentation and ensure regulatory compliance.
• Collaborate with physicians, nurses, and other health professionals to deliver integrated care.
