AdventHealth in Merriam, KS is seeking a dedicated Physical Therapy Assistant to support patient care in home health settings. The role involves collaborating with case managers, therapists, and healthcare professionals to ensure effective discharge planning and continuity of care.

The PTA will monitor patient progress, adapt treatment plans with supervision, and educate patients and families. Requirements include an Associate degree, PTA license, BLS, and a valid drivers license.

Qualifications

• Basic computer skills with Microsoft Office
• Ability to read, write and speak conversational English
• Strong communication and teamwork abilities

Responsabilités

• Maintains open communication with the case manager, primary therapist, and other healthcare professionals to facilitate interdisciplinary continuity of care and discharge planning.
• Maintains productivity standards and adjusts work hours to meet patient needs.
• Participates in departmental meetings, education sessions, and performance improvement initiatives to maintain clinical competencies and evidence-based practices.
• Monitors patient response to treatment and communicates with the Physical Therapist for necessary adjustments.
• Educates patients, families, and caregivers on identified needs and implements treatment plans under the supervision of a Physical Therapist.
• Documents interventions and progress clearly and timely following standards of practice and insurance guidelines.
• Other duties as assigned.

Job Description:

• Flexible scheduling with Full Time, Part Time and PRN available

• Shift: Monday-Friday 8am-4:30pm /On-Call Weekend Commitment Monthly/ Quarterly
• Location: Coverage in NE Johnson County MO and Johnson County KS.
• Other Areas : Jackson, Platte, Clay, Cass Counties in Missouri and/or Leavenworth, Johnson, Wyandotte, Franklin, Douglas and Miami Counties in Kansas.

• Documents interventions and patient progress accurately, completely, and timely, using measurable and objective terminology in line with standards of practice, insurance guidelines, and department/hospital policies and procedures.
• Other duties as assigned.

Knowledge, Skills, and Abilities:

• Entry level competency of Physical therapy practice, theory, principle, and applications as defined in competency department policies. [Required]
• Basic computer skills and knowledge of Microsoft Office. [Required]
• Must be able to read, write, and speak conversational English. [Required]
