Qualifications

  • Current Physical Therapy license and CPR certification required.

Job description

We are hiring a full-time Physical Therapist/PT to join our passionate team in Washington DC!

Responsibilities
  • Evaluate patients' functional status and physical therapy needs, and consult with physicians to develop plans of care.
  • Provide services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  • Observe and report patient responses and any condition changes to the nurse supervisor and/or physician.
  • Treat patients to relieve pain, develop or restore function, and maintain maximum functioning using evidence-based techniques.
  • Instruct patients, families, and healthcare team members about the care plan, limitations, and progress.
  • Visit patients across the service area and nearby agencies as needed.
  • Complete progress notes for each visit and submit them per policy.
  • Conduct evaluations and develop plans of care within state guidelines, reporting outcomes within 24 hours.
  • Participate in staff meetings and committees as requested.
  • Supervise home health aides and PTAs as allowed by state regulation and agency policy.
  • Participate in weekend rotations to provide services as needed.
  • Prepare discharge summaries and periodic patient condition reports.
  • Provide in-service education upon request.
  • Adhere to LHC Therapy Standards of Practice.
  • Maintain and update clinical knowledge and licensure requirements.
  • Perform other duties as assigned.
Education and Experience

Licensure Requirements:

  • Current Physical Therapy license in the state of practice
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and reliable transportation
