Riverside Health System is seeking an experienced Home Health Physical Therapist to join our team in Warsaw. You will perform evaluations, develop evidence-based care plans, and deliver hands-on therapy in patients’ homes to restore function and independence.

The role requires a PT license and a driver’s license with willingness to travel; we offer competitive pay and a $20,000 sign‑on bonus for eligible therapists.

Qualifications

• PT degree required.
• Experience in home health or pediatric settings a plus.
• Strong communication and documentation skills.

Responsabilités

• Assess & treat: perform patient evaluations and design goal‑oriented PT plans.
• Monitor progress: complete timely reassessments and update plans with the care team.
• Collaborate & lead: participate in care conferences with the multidisciplinary team.
• Support & supervise: guide PT assistants and home health aides, oversee supervisory documentation.
• Document & plan: maintain clear progress notes showing skilled intervention.
• Drive outcomes: adapt plans to maximize recovery and independence.
