Job summary

Riverside Health at Home in Warsaw, VA is seeking an experienced Home Health Physical Therapist to evaluate patients in their homes, develop evidence-based plans, and deliver hands-on therapy. Qualifications

Responsibilities

• Perform patient evaluations, determine PT diagnoses, and design goal-oriented plans of care
• Complete timely reassessments and update care plans including OASIS
• Collaborate with physicians and multidisciplinary team to ensure continuity of care
• Supervise and mentor Physical Therapy Assistants and Home Health Aides
• Prepare clear clinical and progress notes reflecting skilled intervention and outcomes
• Adapt treatment plans to maximize recovery and independence
