Job summary

Riverside at Home in Warsaw is seeking an experienced Home Health Physical Therapist to deliver in-home care, perform evaluations, and develop evidence-based plans of care. You will collaborate with an interdisciplinary team to help patients regain independence and mobility while maintaining high reliability in patient experience.

This temporary assignment offers a flat pay rate of $80/hr, with travel using a personal vehicle and opportunities to mentor aides and assistants.

Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy (required).
• 1 year of Physical Therapy experience (preferred).
• Strong understanding of growth and development across the lifespan; assess patient needs, analyze data, and deliver age-appropriate care.
• Excellent communication and documentation skills; collaborative spirit with a passion for patient-centered care.

Responsibilities

• Assess & Treat: Perform patient evaluations, determine PT diagnoses, and design goal-oriented plans of care.
• Monitor Progress: Reassessments, including OASIS assessments, and update care plans with physicians and care team.
• Collaborate & Lead: Work with interdisciplinary team and participate in care conferences to ensure continuity of care.
• Support & Supervise: Guide and mentor PTAs and HHAs, ensuring quality care and supervisory documentation.
• Document & Plan: Prepare clear clinical and progress notes demonstrating skilled intervention.
• Drive Results: Adapt treatment plans to maximize recovery and independence using evidence-based practices.
