Job summary

Riverside Health System in Warsaw, VA is seeking an experienced and compassionate Home Health Physical Therapist to join our Riverside at Home team. In this role you will perform patient evaluations, develop evidence-based plans of care, and deliver hands-on therapy in patients' homes to restore function and independence.

You will collaborate with an interdisciplinary team, supervise aides as needed, document progress, and adapt treatment plans using best practices.

Qualifications

• Education: Degree in Physical Therapy (BS/MS/DPT) required.
• 1 year of PT experience preferred.
• Active Virginia PT license required upon hire; CPR/BLS required within 30 days.

Responsibilities

• Perform patient evaluations and PT diagnoses.
• Develop and implement evidence-based plans of care.
• Complete OASIS assessments and timely re-evaluations.
• Collaborate with physicians and care teams.
• Supervise PTAs and HHAs and document supervision.
• Maintain clear clinical notes and progress documentation.
