Job summary

Beaumont ASHN LLC in Ohio seeks a Physical Therapist I to provide comprehensive skilled services in patients' homes as ordered by the physician. The PT educates patients and families, ensures safety, and maintains compliant documentation.

The role requires at least one year of PT experience (acute setting preferred) and is based in the Kettering area with a Monday–Friday schedule and on-call weekend/holiday rotation. A $10,000 sign-on bonus is offered to eligible candidates.

Qualifications

• Minimum of one year of experience as a PT in an acute setting is required.
• Home care experience is desired.
• Ability to effectively communicate with patients, families, physicians and co-workers.

Responsibilities

• Provides skilled physical therapy in a patient’s home as ordered by the attending physician.
• Observes, records, and reports patient reactions, changes, and progress.
• Performs OASIS assessments to develop an individualized plan of care and makes adjustments as needs change.
• Develops plan of care and treats to relieve pain and restore function.
• Instructs patient and family on proper use of equipment and home safety.
• Completes clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
