Job summary

Summa Health at Home and Hospice in Akron, OH is seeking a Physical Therapist to join our home-based care team. You will provide skilled therapy to patients in their homes under physician orders, partnering with an interdisciplinary team to promote recovery and independence.

Qualifications

• PT license in the state of employment.
• Minimum 1 year of PT experience in acute care setting.
• Home care experience is preferred.
• Excellent communication with patients and families.

Responsibilities

• Provides skilled physical therapy in a patient’s home as ordered by the physician.
• Observes, records, and reports patient progress to the physician and staff.
• Performs OASIS assessments and develops an individualized plan of care.
• Completes initial evaluation within 48-72 hours of referral.
• Develops and adjusts care plans; supports discharge planning and family education.
