Job summary

Kettering Home Care in Kettering, OH is seeking a Physical Therapist I to provide skilled services in patients' homes. You will assess, plan, and implement therapy programs while educating families and ensuring safety and compliance with regulations.

The role requires an active Ohio PT license and a commitment to delivering high-quality patient care. A $10,000 sign-on bonus is offered with a Monday–Friday schedule and occasional weekend/on-call coverage.

Qualifications

• One year of PT experience in an acute setting is preferred.
• Home care experience is desired.
• Valid Physical Therapist license in the state of employment.

Responsibilities

• Provides skilled physical therapy in a patient’s home as ordered by the attending physician.
• Performs OASIS assessments to develop an individualized plan of care and makes adjustments as needs change.
• Completes and maintains clinical documentation per agency policy and Medicare guidelines.
