Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patients' needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops treatment plans, performs interventions to improve patient well-being, and monitors progress.

• Evaluate the patient's functional status and therapy needs, assisting physicians in developing the plan of care
• Observe and report patient responses and condition changes to the supervising nurse or physician
• Treat patients to relieve pain, restore function, and maintain maximum functioning according to standards of practice
• Educate patients, families, and healthcare team members about the care plan, functional limitations, and progress

Education and Experience

Licensure Requirements

• Current Physical Therapy license in Indiana
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
