Job summary

HCA Healthcare is seeking a Physical Therapist for the Home Health West Asheville Team, NC. You will assess patients, design personalized therapy plans, and work with physicians to improve mobility and independence.

You will deliver evidence-based interventions, monitor progress, and document outcomes while educating patients and families on safe, effective home exercise programs. Mentoring staff and collaborating with a multidisciplinary team are also key responsibilities.

Qualifications

• Active physical therapy license or eligibility required.
• BLS certification within 7 days of start date.
• Driver license required.

Responsibilities

• Evaluate patient abilities and develop personalized treatment plans with physicians and care team.
• Deliver evidence-based interventions to restore movement and reduce pain.
• Monitor progress, adjust plans, and document outcomes accurately.
• Educate patients and families on exercise programs and safe practices for independence.
• Collaborate across disciplines for cohesive, patient-centered care and smooth transitions.
• Mentor therapy assistants or students while upholding HCA Healthcare values.
