Job summary

UnitedHealth Group is seeking a Physical Therapist for home health services in the Sylva area. The role involves evaluating patients, developing and implementing plans of care, and coordinating with physicians and the health care team to improve patient outcomes.

The position features both per-visit and hourly compensation with a comprehensive benefits package, reflecting full-time productivity. CPR certification is preferred, with experience in home health a plus.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and plan of care, coordinating with health care team
• Observes and reports patient response to treatment and changes in condition to supervising nurse/physician
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning
• Instructs patient and family/caregivers regarding plan of care, limitations, and progress toward goals
• Visits patients per Plan of Treatment, completes progress notes per policy
• Completes all evaluations and develops PT plan of care within state guidelines
• Reports outcomes of evaluation, goals, and projected frequency of care
• Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition
