The role requires an active Texas PT license, professional liability insurance, a graduate degree in Physical Therapy, and experience in home health or rehab settings. You will educate patients and caregivers and collaborate with clinicians to ensure evidence-based care.

Qualifications

• Active Texas PT license required.
• Graduate degree in Physical Therapy from an accredited program.
• Professional liability insurance coverage.
• Experience in home health or rehabilitative settings preferred.
• Strong ability to adapt care approaches to various patient needs.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust plans as necessary to achieve outcomes.
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and other therapists.
• Conduct home safety assessments and make recommendations to prevent falls or re-injury.
• Deliver compassionate care while empowering patients to achieve independence.
