Job summary

LiveWell Homecare Agency, LLC in Omaha, NE is seeking a Physical Therapist (PT) to join a home health care team dedicated to helping patients recover in their own homes. The role emphasizes clinical excellence, compassionate care, and collaboration with the healthcare team.

You will assess mobility and function, develop and document treatment plans, provide in-home PT services per physician orders, educate patients and caregivers, and coordinate with staff while following state regulations.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR certification and a current driver’s license.

Responsibilities

• Conduct initial and ongoing assessments of mobility, strength, and function.
• Provide written evaluations and treatment plans outlining needs, goals, and progress.
• Deliver physical therapy services following physician orders and maintain documentation.
• Plan discharge care with patient and caregiver education on home exercises.
• Coordinate with the healthcare team and ensure compliance with state regulations.
• Supervise PTA, HHA, and complete assessments as applicable.
