Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health in Illinois. You will assess patient needs, determine treatment plans, and perform interventions to improve mobility and well-being.

Responsibilities include evaluating functional status, collaborating with the care team, documenting progress, and educating patients and families on the plan of care. A valid PT license and driving access are required.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and PT needs, assists in care plan.
• Provides services within scope of practice per state laws and plan of care.
• Observes and reports patient responses to treatment to nurse/physician.
• Treats patients to relieve pain, restore function, and maximize functioning.
• Educates patient and family about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes PT evaluations and develops plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
• Works with diverse patients regardless of background.
