Job summary

Homeside Rehab is seeking compassionate Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to provide in-home physical therapy across New York City and Yonkers, NY. You will evaluate patients, design plans, and deliver evidence-based interventions in a home setting.

Responsibilities include performing evaluations, creating treatment plans, documenting care electronically, and coordinating with physicians and the care team.

Qualifications

• NY State Physical Therapist (PT) or Physical Therapist Assistant (PTA) license is required
• Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or SNF preferred

Responsibilities

• Perform comprehensive PT/PTA evaluations and develop individualized treatment plans
• Provide evidence-based interventions to improve strength, balance, mobility, and functional independence
• Deliver skilled therapy services in patients' homes
• Complete accurate and timely electronic documentation
• Collaborate with physicians and care team to coordinate patient care
• Educate patients and caregivers on home exercise programs and techniques
• Communicate scheduling and patient updates with office staff
