Job summary

All About Home Care is seeking a Physical Therapist (PT) to cover the Upper Central North Greater Houston area, including Conroe, Spring, and The Woodlands. The role requires TX licensure, possible Spanish speaking, and OASIS documentation proficiency using Kinnser software.

The PT will perform initial assessments, generate OASIS documentation, and provide physician-prescribed physical therapy while following agency policies in a flexible, patient-centered setting.

Qualifications

• Graduate from a Physical Therapist curriculum approved by the APTA or AMA-APTA
• Texas license as a Physical Therapist in good standing
• Two years experience preferred
• Spanish speaking a plus
• Ability to complete OASIS visits and documentation

Responsibilities

• Makes the initial assessment visit and generates OASIS documentation the same day
• Understands and adheres to established policies and procedures
• Provides physician-prescribed physical therapy

All About Home Care is a Houston-based home health care agency. We’ve been serving Houston and the surrounding communities since 2004, providing quality home health services. We are currently looking for a Physical Therapist (PT) to help serve patients in the Upper Central North coverage area of Greater Houston, which mainly includes Conroe, Spring, The Woodlands, and surrounding areas. If you have experience in home health and are interested in covering this area, please submit your resume. We use Kinnser software to document patient care.

Qualifications

• Graduate from a Physical Therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association and American Physical Therapy Association
• Currently licensed (licensed physical therapist) in the state of Texas.
• Two years experience preferred.
• Ability to exercise initiative and independent judgment.
• Spanish speaking a plus
• Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care)
