As a Home Health – Physical Therapist (PT), you will be responsible for assessing patients' needs, developing treatment plans, and providing therapy services to help patients regain their mobility and independence.

Responsibilities

• Conduct initial assessments of patients' physical abilities and develop treatment plans
• Provide therapy services to patients in their homes
• Monitor patients' progress and adjust treatment plans as needed
