Job summary

All About Home Care Inc. is a Houston-based home health care agency serving the Greater Houston area since 2003. We are seeking Physical Therapists to cover the East side (Northeast, Southeast, or Central East) of the Greater Houston area.

PRN PTs are also sought for The Woodlands and Conroe. We use Kinnser software to document patient care. The ideal candidate will be licensed in Texas, with 2 years of experience preferred, and Spanish speaking a plus.

Qualifications

• Graduate from a Physical Therapist curriculum approved by the APTA or AMA/ACHE.
• Currently licensed in the state of Texas as a Physical Therapist.
• Two years experience preferred.
• Spanish language ability is a plus.
• Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care).

Responsibilities

• Makes initial assessment visit and generates OASIS documentation the same day.
• Understands and adheres to established policies and procedures.
• Provides physician-prescribed physical therapy.
• Performs evaluation and treatment visits.
• Improves or minimizes residual physical disabilities.
• Returns the patient to optimal and productive level within capabilities.
• Participates with healthcare personnel in patient care planning.
• Direct and supervise personnel as required.
• Performs all skilled procedures as ordered by physician.
• Consults with physicians regarding treatment changes and progress.
• Involves patient families in home programs and activities of daily living.
• Attends patient care conferences and prepares progress notes.

We are looking for Physical Therapists (PTs) to help serve patients on the East side (Northeast, Southeast, or Central East coverage) of the Greater Houston area. Qualifications:

• Graduate from a Physical Therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association and American Physical Therapy Association
• Currently licensed (licensed physical therapist) in the state of Texas.
• Two years experience preferred.
• Ability to exercise initiative and independent judgment.
• Spanish speaking a plus
• Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care)

• Prepares clinical and progress notes daily.
• Assists physicians with evaluating level of function.
• Helps develop the plan of care and revise as necessary.
• Consults with family and agency personnel.
