Job summary

ABL Health Care in Martinez, CA is seeking a Full Time Physical Therapist (PT) to join our Home Health team. The PT will evaluate patients, create and implement care plans, and monitor progress in patients' homes, collaborating with our Clinical Director and staff.

The role offers a supportive work environment, back-office support, company devices, and competitive benefits. Travel within a 15–25 mile radius is required.

Qualifications

• Current PT license to practice in California required.
• Experience in home health or hospice care preferred (1 year).
• Valid California driver's license and ability to travel within a 15–25 mile radius.

Responsibilities

• Provide and oversee skilled intermittent home health care.
• Evaluate patient status and adjust care plans.
• Administer treatments and monitor conditions.
• Develop and revise the patient's plan of care (POC).
• Address problems identified in the POC.
• Educate patient, family, and caregivers.
• Communicate with treating disciplines as needed.
• Maintain accurate clinical records using company device.
• Adhere to company policies and ensure high standards of care.
