Job summary

Qualifications

• Active Physical Therapist (PT) license in Missouri and Kansas.
• Minimum of one year of experience as a Physical Therapist.
• Clinical expertise to deliver high-quality patient care.
• Current driver’s license and reliable transportation.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team.
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently while maintaining strong communication with the support team.
• Travel to patient homes within the assigned territory.
