Job summary

United Home Health Care is seeking a licensed Physical Therapist to deliver one-on-one care in patients’ homes. You will assess, treat, and design individualized plans to restore mobility and safety, while working autonomously on a flexible schedule.

Responsibilities include manual therapy, gait training, and coordination with a multidisciplinary team. Travel to patient homes is required, with documentation in an EMR system and focus on patient education for home exercises and fall prevention.

Qualifications

• Valid State PT license.
• Home health experience preferred.
• Ability to travel to patient homes.

In this role, you won't just be treating symptoms; you'll be helping patients regain their independence in the place they feel most comfortable—their own home.

What You'll Do

• Restore Independence: Conduct comprehensive initial evaluations and develop personalized plans of care focused on functional mobility and safety.
• Clinical Excellence: Provide high-quality manual therapy, therapeutic exercise, and gait training tailored to the home environment.
• Collaborative Care: Coordinate with physicians, occupational therapists, and nursing staff to ensure a holistic approach to patient recovery.
• Documentation: Maintain timely and accurate clinical records (OASIS experience is a major plus!) via our electronic medical record system.
• Education: Empower patients and their families by teaching home exercise programs and fall prevention strategies.

What We’re Looking For

• Credentials: Valid State License in Physical Therapy (PT).
• Experience: Home Health experience preferred, but we are willing to train the right "hospital-to-home" transitioner.
• Independence: A self-starter who can manage their own schedule and travel to various patient homes.
• Communication: Strong interpersonal skills to build rapport with patients and families.
• Reliability: Valid driver's license, reliable transportation, and auto insurance.
