All About Home Care Inc., a Houston-based home health agency, is seeking Physical Therapists to serve patients in the East side of Greater Houston with PRN opportunities in The Woodlands and Conroe. Applicants should be licensed in Texas and have PT credentials, with home health experience preferred and Spanish speaking a plus.

• Graduate from a Physical Therapist curriculum approved by the American Physical Therapy Association or related accrediting bodies
• Currently licensed (licensed physical therapist) in the state of Texas
• Two years experience preferred
• Spanish speaking a plus
• Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care)

Responsabilidades

• Makes the initial assessment visit and generates OASIS documentation the same day of the visit
• Provides physician prescribed physical therapy
• Performs evaluation and treatment visits
• Implements plan of care and updates as needed
• Collaborates with physicians and family to optimize patient outcomes
• Supervises staff as required
• Participates in performance improvement activities
• Completes daily progress notes and relevant documentation

Job Description:

All About Home Care is a Houston-based home health care agency. We’ve been serving the Houston and surrounding communities since 2004, providing quality home health services. We are looking for Physical Therapists (PTs) to help serve patients on the East side (Northeast, Southeast, or Central East coverage) of the Greater Houston area. We are also seeking PRN PTs for The Woodlands and Conroe areas. If you have any experience in home health, please submit your resume. We use Kinnser software to document patients care

Qualifications:

• Graduate from a Physical Therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association and American Physical Therapy Association
• Currently licensed (licensed physical therapist) in the state of Texas.
• Two years experience preferred.
• Ability to exercise initiative and independent judgment.
• Spanish speaking a plus
• Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care)

Responsibilities:

• Makes the initial assessment visit and generates OASIS documentation the same day of the visit
• Understands and adheres to establish policies and procedures.
• Provides physician prescribed physical therapy.
• Performs evaluation and treatment visits.
• Improves or minimize the residual physical disabilities of the patient.
• Returns the individual to optimal and productive level within the patient’s capabilities.
• Participates with all other healthcare personnel in patient care planning.
• Direct and supervise their personnel as required.
• Performs all skilled procedures as ordered by physician.
• Consults with physicians regarding change in treatment ,reports to physicians regarding patients progress, instructs patients and families/significant others in home programs and activities of daily living.
• Participates in in-service programs and presents in service programs as assigned.
• Participates in performance improvement activities as assigned.
• Attends all patient care conferences as scheduled.
• Prepares clinical and progress notes daily.
• Assists physicians with evaluating level of function.
• Helps develop the plan of care and revise as necessary.
• Consults with family and agency personnel.
