Job summary

All About Home Care Inc., a Houston-based home health care agency, seeks a Physical Therapist Assistant (PTA) to support patients in the Southwest Greater Houston area, including Katy, Sugar Land, Bellaire, Richmond, Rosenberg, Fresno, Missouri City and surrounding communities. PRN PTAs are also needed for The Woodlands and Conroe.

Likely responsibilities include home-based therapy under a PT plan of care, evaluation support, patient education, and collaboration with a broader clinical team

Qualifications

• Graduate from a two-year college program approved by the American Physical Therapy Association (APTA).
• Licensed to practice as PTA in Texas at present.
• Two years of home health experience preferred.

Responsibilities

• Provides physician prescribed physical therapy under a plan of care established by a PT.
• Takes initial history and performs initial evaluation.
• Adheres to established policies and procedures.
• Improves or minimizes residual disabilities for patients.
• Educates patients and family on home programs and activities of daily living.
• Documents care and updates care plans for each patient visit.

Job Description:

All About Home Care is a Houston-based home health care agency. We’ve been serving Houston and the surrounding communities since 2004, providing quality home health services. We are currently looking for Physical Therapist Assistant (PTA) to help care for patients in the Southwest part of Greater Houston including areas such as Katy, Sugar Land, Bellaire, Richmond, Rosenberg, Fresno, Missouri City, and surrounding areas. We are also seeking PRN PTAs for The Woodlands and Conroe areas. We use Kinnser software to document patient care.

Qualifications:

• Graduate from a two-year college level program approved by the American Physical Therapy Association
• Currently licensed in the state of Texas
• Two year of experience in Home Health preferred

Responsibilities:

• Understands and adheres to established policies and procedures
• Provides physician prescribed physical therapy under a plan of care established by a PT
• Improves or minimizes residual physical disabilities of patients
• Returns the individual to optimum and productive level within patient’s capabilities
• Performs all skilled procedures as ordered by physician and according to the plan of care established by a PT
• Participates with all other health care personnel in patient care planning
• Takes initial history and perform initial evaluation
• Consults with PT regarding change in treatment
• Instructs patients and family/significant others in home programs and activities of daily living
• Participates in mandatory in-service programs and presents in-service programs as assigned
• Participates in performance improvement activities as assigned
• Attends all patient care conference as scheduled
• Prepares medical records, progress notes, and updates care plans for each patient visit in a timely manner as per Agency
