All About Home Care Inc., a Houston-based home health care provider, seeks licensed Physical Therapists to cover the Southeast Greater Houston area including Baytown, Pasadena, and nearby communities. You will perform initial assessments, create and manage OASIS documentation, and deliver physician-prescribed PT services in patients’ homes.

Ideal candidates have Texas licensure, a PTA/APTA-approved PT program, and at least 2 years of experience; Spanish-speaking is a plus.

Qualifications

• Graduate from PT curriculum approved by APTA/Committee on Allied Health Education and Accreditation
• Currently licensed Physical Therapist in Texas
• Two years experience preferred
• Spanish speaking a plus
• Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care)

Responsabilités

• Makes the initial assessment visit and generates OASIS documentation the same day
• Provides physician-prescribed physical therapy and treatment
• Performs evaluation and treatment visits to improve patient outcomes
• Helps develop and revise the plan of care with physician input
• Communicates progress and home program guidance to patients and families
• Participates in care planning with the healthcare team and supervises staff as needed

All About Home Care is a Houston-based home health care agency. We've been serving Houston and the surrounding communities since 2003, providing quality home health services. We are currently looking for Physical Therapists (PTs) to help serve patients in the South East area of Greater Houston, including Baytown, Pasadena, Clear Lake, Pearland, Friendswood, Texas City, League City, and surrounding areas. If you have experience in home health and are interested in covering this area.

We use Kinnser software to document patient care.

Qualifications:

• Graduate from a Physical Therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association and American Physical Therapy Association
• Currently licensed (licensed physical therapist) in the state of Texas.
• Two years experience preferred.
• Ability to exercise initiative and independent judgment.
• Spanish speaking a plus
• Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care)

Responsibilities:

• Makes the initial assessment visit and generates OASIS documentation the same day of the visit
• Understands and adheres to establish policies and procedures.
• Provides physician prescribed physical therapy.
• Performs evaluation and treatment visits.
• Improves or minimize the residual physical disabilities of the patient.
• Returns the individual to optimal and productive level within the patient's capabilities.
• Participates with all other healthcare personnel in patient care planning.
• Direct and supervise their personnel as required.
• Performs all skilled procedures as ordered by physician.
• Consults with physicians regarding change in treatment ,reports to physicians regarding patients progress, instructs patients and families/significant others in home programs and activities of daily living.
• Participates in in-service programs and presents in service programs as assigned.
• Participates in performance improvement activities as assigned.
• Attends all patient care conferences as scheduled.
• Prepares clinical and progress notes daily.
• Assists physicians with evaluating level of function.
• Helps develop the plan of care and revise as necessary.
• Consults with family and agency personnel.
