Job summary

Home Sweet Home is seeking a licensed Physical Therapist for home health in the Phoenix West Valley region. Per visit compensation with an annual earning potential up to $103,500 and a $5,000 sign-on bonus. Schedule includes Per Diem, Part-Time, and Full-time options.

Qualifications

• Active Arizona Physical Therapist license.
• CPR certification (AHA).
• Minimum 1 year of clinical experience (home health preferred).
• Strong clinical judgment and time management.
• Reliable transportation, valid driver’s license, and insurance.
• Strong communication and a patient-first approach.

Responsibilities

• Conduct in-home evaluations, start of care, and follow-up visits
• Develop and implement individualized treatment plans
• Improve strength, balance, mobility, and functional independence
• Educate patients and caregivers on safety and equipment use
• Document visits using WellSky Kinnser EMR
• Collaborate with physicians, RNs, and the care team
