Job summary

Home Sweet Home is seeking a licensed Physical Therapist to provide one-on-one home health care in the Phoenix area. You will conduct in-home evaluations, start of care, and follow-up visits, then develop targeted treatment plans to improve strength, balance, mobility, and independence.

This role offers flexible scheduling (per diem, part-time, and full-time) with per-visit compensation and a sign-on bonus of $5,000.

Qualifications

• Active Arizona Physical Therapist license
• CPR certification (AHA)
• Minimum 1 year of clinical experience (home health preferred)
• Strong clinical judgment and time management
• Reliable transportation, valid driver’s license, and insurance
• Strong communication and a patient-first approach

Responsibilities

• Conduct in-home evaluations, start of care, and follow-up visits
• Develop and implement individualized treatment plans
• Improve strength, balance, mobility, and functional independence
• Educate patients and caregivers on safety and equipment use
