Job summary

Brooks Rehabilitation is seeking a PRN Physical Therapist for home health in Volusia County. You will participate in evaluation, planning, and implementation of therapy programs to restore function and independence. Supervising therapists and aides may be required as part of the role.

You will assess patient status, design coordinated treatment plans, document progress, and prepare patients for discharge with home programs and caregiver guidance. A Florida license and BLS are required.

Qualifications

• Licensed in the state of Florida.
• Hands-on BLS certification.

Responsibilities

• Evaluate and treat patients using PT interventions to restore function.
• Develop and adjust treatment plans with patient/family input.
• Supervise PTAs, aides, and interns as applicable.
• Document evaluations, progress notes, and discharge summaries.
• Coordinate care with physicians and rehabilitation team.
• Prepare patients for discharge with home programs and caregiver instruction.
