Job summary

Brooks Rehabilitation is seeking a Physical Therapist for Home Health in Jacksonville on a PRN basis. The PT will evaluate, plan, implement, and monitor patient care to restore physical function, and may supervise PTAs and aides as needed.

Responsibilities include modifying treatment plans, coordinating with physicians, and ensuring proper patient and family education for discharge. Qualified candidates will be licensed in Florida, hold a current BLS card, and demonstrate strong communication,

Qualifications

• Licensed to practice physical therapy in Florida.
• Hands-on BLS certification.
• Maintains patient confidentiality.
• Adheres to universal precautions when delivering patient care.
• Exhibits courtesy, compassion, and respect for all patients and co-workers.
• Maintains a clean and safe environment; identifies hazards.
• Adheres to attendance, dress code, and conduct policies.

Responsibilities

• Assess and plan patient care and treatment.
• Develop and adjust treatment programs based on patient needs and goals.
• Consult with physicians and rehab team to design a coordinated plan.
• Provide PT interventions and supervise related staff when applicable.
• Prepare discharge instructions and patient/family education.
• Document evaluations, progress notes and discharge summaries.
• Manage schedule to meet productivity expectations.
• Attend staff meetings and in-services.
