Amedisys is seeking a compassionate Physical Therapist for a PRN on-call role in home health. You will deliver patient-centered therapy at the patient’s home, focusing on movement, mobility, and independence.

Flexible coverage for overflow and PTO, with ongoing assessments and discharge planning. Qualified candidates hold a state license, CAPTE-accredited program, CPR certification, and at least 1 year of related home health experience or equivalent mentoring.

Formación

• Current license to practice physical therapy in the assigned state.
• Graduate from CAPTE-accredited PT program.
• At least 1 year of related PT experience for home health care; otherwise approval with mentoring.
• Current CPR certification.

Responsabilidades

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations (every 30 days), modify interventions, and perform discharge planning.
• Other duties as assigned.
